Required Immunizations
These charts provide guidance for parents about minimum required vaccinations a child must have to enter or remain in school.  Please consult your family doctor or Health Department for more information or questions you may have.

Varicella Disease (Chickenpox) Form
This form must be completed only if a student has had varicella (chickenpox) disease.

Parent/Guardian Parent Portal Agreement
This form must be complete for parental access to PowerSchool Parent PortalPowerSchool Parent Portal allows a parent to view students grades, attendance as well as communicate directly with their child's teacher.

Schools of Choice Application
This form must be completed by non-resident students who are applying to attend Richmond Community Schools through the Schools of Choice program.  Applications for Fall enrollment are accepted from April 1st through the Thursday after Labor Day.  All Schools of Choice students must be registered and attending school by the Friday following Labor Day.

Indian Education Grant Consortium
L'Anse Creuse Public Schools has formed a consortium in conjunction with Chippewa Valley Schools, New Haven Community Schools, and Richmond Community Schools called the Indian Education Grant Consortium.  This program will be available to eligible students in these four school districts.  If your child has a parent or grandparent that has American Indian ancestry, please complete the form and return it with your registration paperwork.

Kindergarten Age Waiver Request
Beginning with the 2015-16 school year, students entering kindergarten must be five (5) on or before September 1st.  If a students turns five (5) on or before December 1st, the parent/guardian may request their child attend kindergarten by completing the Kindergarten Age Waiver Request form.

Residency Verification
This form must be completed for families that are currently living with family or friends (e.g. often referred to as "Doubling-up").  The owner or renter of the home must be present when registering and completing this form.

Volunteer Release Form (I-CHAT)
This form must be completed by any adult who may wish to volunteer in the classroom, attend field trips, assist with school activities, including athletics, etc.  This information obtained by this report is kept confidential in the Superintendent's Office.  Should an adult's background check be returned with a finding, the Richmond Community Schools will be in contact with the individual.

Record Release
This form must be completed for all students who attended another school prior to Richmond Community Schools.  This form allows the school district to obtain the student's prior records, including special education records.

Authorization for Prescribed Medication or Treatment
This form must be completed for any students to use prescribed medication or receive a prescribed treatment in school.  School district personnel shall not administer any medical or treatment without this form properly filled out by the parent and student's physician.

Authorization for Non-Prescribed Medication and Treatment
This form must be completed for any students, grade 5-12, to use non-prescribed medication or receive a non-prescribed treatment in school.  School district personnel shall not administer any medication or treatment without this form properly filled out by the parent/guardian.

Pay to Participate Information
Richmond Community Schools has implemented a Pay to Participate policy for all athletic sports at the high school and middle school level.

Pesticide Notification Advisory to Parents
Richmond Community Schools utilizes an Integrated Pest management (IPM) approach to control pests.  This form only needs to be completed if you would like to be notified of pesticide applications in the District.

Macomb County Health Department Hearing and Vision Program
Macomb County Health Department offers a free of charge hearing and vision test for those students entering Kindergarten during certain times of the year.

Free and Reduced Price School Meals
Click on the link below to fill out an application for free and reduced price school meals.  Only one application per household is necessary, even if your children attend more than one school in Richmond Community Schools.  The application must be filled out completely to certify students for free and reduced price school meals.

School-of-Choice Bus Stop
Are you a Schools of Choice family?  Richmond community Schools has identified eight (8) current bus stops that resident students get on and off the bus at.  If you would like transportation, email Deb Dobbs, Transportation and Dispatch Coordinator, at ddobbs@Richmond.k12.mi.us, and request one of the following bus stops for your student(s):
